Speaking of surveys.. I went to Bob Evans last weekend. The food was great, but the service was horrible. For the first time ever, i decided to call that 800 number survey they put on your reciept. I was shocked to see that only 1 question was about the server!

How do you decide which questions will get you actionable feedback to improve your services?
